Q. Why is metabolism so important?

I wrote the last line of my first book, Torch, and then spent an hour crying while lying on a cool tile floor in a house on a hot Brazilian island. After I finished my second book, Wild, I walked alone for miles under a clear blue sky on an empty road in the Oregon Outback. I sat bundled in my coat on a cold patio at midnight staring up at the endless December stars after completing my third book, Tiny Beautiful Things. There are only a handful of other days in my life--my wedding, the births of my children--that I remember as vividly as those solitary days on which I finished my books. The settings and situations were different, but the feeling was the same: an overwhelming mix of joy and gratitude, humility and relief, pride and wonder. After much labor, I'd made this thing. A book. Though it wasn't technically that yet.

The real book came later--after more work, but this time it involved various others, including agents, publishers, editors, designers, and publicists, all of whose jobs are necessary but sometimes indecipherable to me. They're the ones who transformed the thousands of words I'd privately and carefully conjured into something that could be shared with other people. "I wrote this!" I exclaimed in amazement when I first held each actual, physical book in my hands. I wasn't amazed that it existed; I was amazed by what its existence meant: that it no longer belonged to me.

Two months before Wild was published I stood on a Mexican beach at sunset with my family assisting dozens of baby turtles on their stumbling journey across the sand, then watching as they disappeared into the sea. The junction between writer and author is a bit like that. In one role total vigilance is necessary; in the other, there's nothing to do but hope for the best. A book, like those newborn turtles, will ride whatever wave takes it.

It's deeply rewarding to me when I learn that something I wrote moved or inspired or entertained someone; and it's crushing to hear that my writing bored or annoyed or enraged another. But an author has to stand back from both the praise and the criticism once a book is out in the world. The story I chose to write in Wild for no other reason than I felt driven to belongs to those who read it, not me. And yet I'll never forget what it once was, long before I could even imagine how gloriously it would someday be swept away from me.

Cokie Roberts: You are a journalist. Has writing about ALS helped you deal with it? How in general has writing shaped you?

Susan Spencer-Wendel: As a journalist, I am a truth teller, no matter how uncomfortable it may be. There is no pretending. This helped me a great deal to recognize the reality of ALS. I have written thousands of stories as a newspaper reporter, which prepared me. One, because I’ve seen all kinds of tragedy in my career. I realized bad things happen to good people on quite a regular basis. And, two, because I knew how to distill events into stories. Writing is wonderful because it forces us to crystallize our jumbled thoughts via words.

CR: When it came to literally writing this book you faced some physical challenges. Can you tell us how you ended up writing the book?

SS: With ALS, the tiniest muscles fail first. Like the ones in your fingers. I was no longer able to type on a regular keyboard. I did not have the strength in my finger to press the keys. So I began on an iPad. Typing each letter on the touchscreen. Then my hand began to drag across the screen. Right about then, I landed a book deal, which was a tremendous incentive to improvise. I had always used the notes function on my iPhone to jot things down. Phrasing which came to me, grocery lists, or on trips, when getting out a laptop was not possible. I loved knowing that it was always there. I'd write anywhere and everywhere. As I waited for the children at music lessons, etc. I held the phone in my left hand. Propped it flat atop my curled fingers, and used my right thumb to type 89,000 words in four months. One letter at a time. Such is the power of desire. On the phone, I can only see 7 lines at a time. Which actually is a blessing. To hyperfocus on the sentence at hand. Active verbs. Few adjectives. I had an outline, a recipe, to create the book. An absolute must for a book, I believe. Bret Witter helped so with that. He saw the full picture, the book, when I saw only a tiny screen. He reminded me where I was at when I was absolutely awash in memories.

CR: You write about looking at life joyfully. Is that what you want readers to take away from this book—that even in rough situations they can experience great joy?

SS: Of course. I am not trying to be Dr. Phil, but I do hope it will inspire people to set their intention to do so. And make them feel lucky...

CR: Is that message especially important for your children to hear?

SS: Absolutely. I hope they learn "My ma did not feel sorry for herself. So neither will I."

CR: Many people with your diagnosis would either crawl into a cave or go from doctor to doctor trying to survive a little longer. You chose not to do that. Why?

SS:The problem with a cave is it has no windows. And the problem with knocking on umpteen doctor doors is there is nothing behind the door. There is no cure. Period. I'm realistic. A cure, a drug, won't come in time for me. I am not giving up. I am accepting. There is a difference. It was a chain reaction in my mind. I am not sure what set off what. Like an orchid lying fallow, neglected in my side yard, and one day a bloom appears. The factors were just there. Nature itself was a factor for me. Nature is so perfect. Photosynthesis. Pollination. Conception (Huge smile!). The monarch butterflies which migrate thousands of miles to Mexico to convene there. The rainforest canopy, where ants can fly so they don't fall to the ground. The laws of physics. That objects in motion have energy. I don't have the will or motion and, ergo, the energy to fight nature, beautiful nature. Also a major factor is my husband, John. I so want him to have a chance at another life. Not saddled with the weight of an invalid wife. Desire is root of all suffering, I believe. To want something you can't have. The cure is to not want it. I practice not wanting a cure, preparing to die. Choosing the path of least resistance. Going gracefully into the night.

What about your children??? People wonder.

Remember, they are MY children. I know what is best for them. I know that they are well-provided for. I know the love that surrounds them. I know the more gracefully I accept things, so too may they. I know they have my strength. I have not wavered once in these beliefs.

CR: Your therapy—travel—was not an easy one to accomplish. You had to go through tough things physically in order to see some of those faraway places. Were the trips worth the sometimes scary situations you found yourself in?

SS: Absolutely. I would not use the word "scary" though. We weren't swimming with sharks. When I was exhausted at the bottom of the stairs at Wreck Beach, I wasn't scared. More just worried. I adore traveling. So it was natural to go, not difficult to accomplish at all. But not without the help of John, Nancy and Steph. If they sound like saints in the book, that's cause they are saints.

Since that day, with the industry in pursuit of its win-at-all-costs strategy, the situation has only grown more dire. Every year, the average American eats thirty-three pounds of cheese (triple what we ate in 1970) and seventy pounds of sugar (about twenty-two teaspoons a day). We ingest 8,500 milligrams of salt a day, double the recommended amount, and almost none of that comes from the shakers on our table. It comes from processed food. It’s no wonder, then, that one in three adults, and one in five kids, is clinically obese. It’s no wonder that twenty-six million Americans have diabetes, the processed food industry in the U.S. accounts for trillion a year in sales, and the total economic cost of this health crisis is approaching 0 billion a year.

Q. How did you land on salt, sugar, and fat as your way to write about the industry? Why these three ingredients?

A. I’d been investigating a surge in deadly outbreaks of E. coli in meat when an industry source, a microbiologist, suggested that if I wanted to see an even bigger public health hazard, I should look at what food companies were intentionally adding to their products, starting with salt. And sure enough, when I looked at this--by gaining access to high level industry officials and a trove of sensitive, internal records--a window opened on how aggressive the industry was wielding not only salt, but sugar and fat, too. These are the pillars of processed foods, the three ingredients without which there would be no processed foods. Salt, sugar and fat drive consumption by adding flavor and allure. But surprisingly, they also mask bitter flavors that develop in the manufacturing process. They enable these foods to sit in warehouses or on the grocery shelf for months. And, most critically to the industry's financial success, they are very inexpensive.

Q. So, how big is the processed food industry, exactly? What kind of scale are we talking about here?

A. Huge. Grocery sales now top trillion a year in the U.S., with more than 300 manufacturers employing 1.4 million workers, or 12 percent of all American manufacturing jobs. Global sales exceed trillion. But the figure I find most revealing is 60,000: That’s the number of different products found on the shelves of our largest supermarkets.

Q. How did this get so big?

A. The food processing industry is more than a century old--if you count the invention of breakfast cereals--so it’s been steady growth. But things really took off in the 1950s with the promotion of convenience foods whose design and marketing was aimed at the increasing numbers of families with both parents working outside the home. The industry's expansion, since then, has been entirely unrestrained. While food safety is heavily regulated, the government has been industry's best friend and partner in encouraging Americans to become more dependent on processed foods.

Q. What three things should a health-conscious supermarket shopper keep in mind?

A. The most alluring products--those with the highest amounts of salt, sugar and fat--are strategically placed at eye-level on the grocery shelf. You typically have to stoop down to find, say, plain oatmeal. (Healthier products are generally up high or down low.) Companies also play the better-nutrition card by plastering their packaging with terms like "all natural," "contains whole grains," “contains real fruit juice,” and "lean," which belie the true contents of the products. Reading labels is not easy. Only since the 1990s have the manufacturers even been required to reveal the true salt, sugar, fat and caloric loads of their products, which are itemized in a box called the "nutrient facts." But one game that many companies still play is to divide these numbers in half, or even thirds, by reporting this critical information per serving--which are typically tiny portions. In particular, they do this for cookies and chips, knowing that most people can't resist eating the entire three-serving bag. Check it out sometime.
